Information Needed For Tax Computation
In order to properly compute your return, there are several items of information I will need:
E-mail address: Over the last few years, I've found this to be an extremely effective means of communication, eliminating phone tag, missed answering machine messages, etc. Please try to use this if possible.
If you are a first time filer with me, a copy of last year's return
Your name (first, MI, last), date of birth, and social security number
Your spouse's name, date of birth, and social security numberPlease note: A woman who marries and changes her last name should notify the Social Security Administration immediately. The IRS is increasing it's name/number matching. There is a distinct possibility of a reduced refund or a request for additional amount due until the SSA is notified of the name change.
All dependent's names, dates of birth, and social security numbers
The county and school district you reside in
W-2's
1099's for interest and dividend income, including tax exempt
1099-R's for retirement income
Social Security Administration statements, if applicable
1099-R's for IRA distributions
Any paperwork concerning IRA contributions
1099-B's and all information applicable to buying or selling stocks, bonds, or mutual fund transactions. This must include the date(s) you bought, and the price(s) you paid for the security.
Small business information (Schedule C Sole Proprietorships)
Rental property income and expense information
Unemployment Compensation
Schedule K-1's from estates, trusts, partnerships and 1120S (subchapter S) corporations
Other Income, including 1099-G's (gambling income)
Child Day Care: provider's name, address, social security or employer identification number, and amount paid for each dependent. For filers who have cafeteria plans that provide for day care expenses, the total (reimbursed plus out-of-pocket) expenditure is required.
For MO and KS residents age 65 or older: You may qualify for a state tax refund even if you're not required to file a return. This is for taxpayers on a fixed income, and includes $200 per person prescription credit, and a percentage of real estate tax or rent payments (MO), or a $60 per person food tax credit (KS).
